A well-established, not-for-profit hospice provider proudly serving South Jersey since 1980 is hiring a full-time Hospice RN Case Manager across Camden, Burlington, and Gloucester Counties. This is a field-based role where you manage your own caseload — assessments, care plans, symptom management, and family education — for patients and families navigating one of life's most difficult chapters.

What you'll be doing:

      Conducting patient assessments and building individualized care plans

      Managing symptoms and coordinating physician orders

      Educating patients and families on care needs and what to expect

      Collaborating with social workers and IDT members

      Managing your home visit schedule independently and efficiently

What you bring:

      Active NJ RN license (BSN preferred)

      Minimum 2 years of nursing experience

      Hospice or home health background preferred

      Bilingual is a plus!

$90,000 - $98,000 Full-time
