About the Role
As a Home Care & Hospice Physical Therapist, you’ll be more than a clinician—you’ll be a lifeline. You’ll help patients regain strength after surgery, manage chronic conditions, or maintain mobility during hospice care. Your work will empower patients to move with greater ease, reduce pain, and preserve independence, all while providing emotional support to them and their families during deeply personal moments.
What You'll Do
- Conduct in-home physical therapy evaluations to assess mobility, strength, and functional needs.
- Develop and implement individualized care plans that align with each patient’s goals, medical condition, and home environment.
- Provide hands-on therapy to improve balance, coordination, and strength, using adaptive techniques and equipment as needed.
- Educate patients and caregivers on safe mobility practices, fall prevention, and home exercise programs.
- Collaborate closely with nurses, occupational therapists, social workers, and physicians to ensure holistic, coordinated care.
- Document patient progress and adjust treatment plans to reflect evolving needs and goals.
- Offer compassionate support to patients and families navigating the emotional aspects of illness, recovery, or end-of-life care.
Qualifications
- Current state licensure as a Physical Therapist (DPT or MPT).
- Experience in home health, hospice, or geriatric care is strongly preferred.
- Strong clinical assessment skills and the ability to adapt treatment plans to varied home settings.
- Excellent communication skills, with the ability to educate and reassure patients and families.
- A valid driver’s license and reliable transportation for home visits.
- A compassionate, patient-centered approach and a commitment to preserving dignity and quality of life.
